Client Support Services
 

Each diabetic is different and each case is looked at one-by-one to create a care management plan that will work for the client. The DRC attempts to get each person in long term assistance or insurance by facilitating the enrollment process of federal, state, national and local programs.
 

The Diabetes Resource Center can answer your self-management questions and give support.  Our staff is up to date on new glucose meters and testing information. The DRC staff  is aware of Indiana and Federal laws regarding diabetics and are current on Medicare and Medicaid supply coverage. 

The Diabetes Resource Center also has many resources that will save you money on your medication and supplies.  Please check out our resource page.

With help from physicians across St. Joseph County, the Diabetes Resource Center has been able to place on average 200 people into patient assistance programs a year.  This allows local diabetics to get their medications and supplies directly from the pharmaceutical company who manufactures it bringing in over $500,000 worth of medication to St. Joseph County each year.  Each company has their own rules and eligibility standards so please contact the Client Services Manager if you need a referral or assistance at 574-314-5422. 

 


 

Financial Assistance Program
 

The Diabetes Resource Center has a limited amount of funding for buying emergency medication for diabetics in St. Joseph County.  As of July 1, 2015 the Diabetes Resource Center will discountinue its co-pay assistance program at pharmacies.  It will continue to provide insulin samples, coupons and certain supplies from the office. Items are available if they are in stock and the person is meets program requirements. 

In 2015, the Diabetes Resource Center was able to directly provide over $39,000 worth of medications and supplies to diabetics.

INDIANA WISEWOMAN is a state program run by United Health Services and the Diabetes Resource Center in north central Indiana.

Diabetes Education
              
The Diabetes Resource Center has diabetes education materials available for pick up at the office.  If you call ahead, a packet based upon your interest will be prepared.  We also have some books & cookbooks which can be checked out of our collection.

For employers: The DRC is able to provide education lunch and learns or programs for your employees on a variety of topics from diabetes, cardiac issues, diet or weith loss.  Please contact us for a list of topics and prices.

Periodically the DRC will have free seminars throughout the community open to the public. You will find them posted on the United Health Services home page or you can be put on a list to be contacted when they happen by emailing us your request.


Labs and Testing

Knowing your numbers is an important part of diagnosing and controlling your diabetes. We are a certified lab site through the FSSA of Indiana and CMS. We cannot do sliding fee scale on labs, but we do offer a $5 reduction in price if you are referred by a medical professional using our referral form. Referral forms should be sent by physicians, medical professionals, and case/social workers.

Labs Offered:
    - Fasting Glucose
          $10.00 (unless at a free event)
    - Hemoglobin A1c
          $35.00
    - Lipid Panel (Total Cholesterol, Triglycerides, LDL, HDL, LDL/HDL Ratio, with a fasting glucose)
          $65.00  

Businesses scheduling labs for employees should inquire about a multiple test discount.
