Diabetes Education

               
The Diabetes Resource Center has multiple diabetes education programs to make sure everybody gets the opportunity to learn about their disease. 

-    One-on-one education at our facility taught by a registered nurse with a background in diabetic and cardiac care

            o        Sliding Fee Scale

Labs and Testing

Knowing your numbers is an important part of diagnosing and controlling your diabetes.  We are a certified lab site through the FSSA of Indiana.  We cannot do sliding fee scale on labs, but we do offer a reduction in price if you are referred or have a physician send a referral for the tests.  Referral forms should be sent by physicians, medical professionals, and case/social workers.

Labs Offered:
     - Fasting Glucose
          $7.50 (unless at a free event)
     - Hemoglobin A1c

          Without Referral $25.00     With $20.00
     - Lipid Panel (Total Cholesterol, Triglycerides, LDL, HDL, LDL/HDL Ratio, with a fasting glucose)

          Without Referral $50.00     With $40.00

Companies & businesses buying wellness tests for employees should inquire about a multiple test discount.

 

Client Support Services
 

Each diabetic is different and each case is looked at one-by-one to create a care management plan that will work for the client.  We try to get each person in long term situations by helping with the enrollment process of state, national and local assistance programs.
 

The Diabetes Resource Center also can answer your self-management questions and give support.  Our staff is up to date on new glucose meters and testing information.  We are aware of Indiana and federal laws regarding diabetics and stay up to date on Medicare and Medicaid supply coverage.

With help from physicians across St. Joseph County, the Diabetes Resource Center has been able to place over 200 people into patient assistance programs.  This allows local diabetics to get their medications and supplies straight from the pharmaceutical company who makes it bringing in over $500,000 worth of medication to St. Joseph County each year.  Each company has their own rules and eligibility standards so please contact the Client Services Specialist if you need a referral or assistance.

Financial Assistance Program
 

The Diabetes Resource Center provides financial assistance to St. Joseph County, Indiana residents with an income level of under 200% poverty.  Each year we are able to stop emergency visits and hospital stays by supplying vital medication to patients. If you or a loved one is in need of assistance, please call the Client Services Specialist to see if you qualify for this program.
