To create and offer services that contribute to a more healthful way of life for the community in which we live and the people we serve.

our programs

Our programs are designed to assist individuals with diabetes and their families in managing chronic disease. Focusing on the delay or elimination of complications through self-care enables individuals to remain productive and participate fully in life’s activities.

services

Our Client Advocate evaluates client needs and assists individuals in finding community resources and services. The Advocate will also help with government forms and prescription assistance programs for free medications.

Financial Assistance Program helps to defray the cost of medication or supplies for those who meet income qualifications.

Free Equipment Loan and Supplies are available from the Association. Durable medical equipment can be loaned. Supplies for the management of diabetes are available to individuals who need them.

Diabetes Education is available through our sister agency, the Diabetes Education Center.
